Costs

$125.00 is the cost on reviewing your medical history form from my website and a ½ hour phone consultation. Payment is done at the time of the phone call with the use of a credit card. All medications will be called in to a pharmacy and mailed to your home. You, of course, pay the pharmacy.

$45.00 is the charge for the 6month follow-up. Again, done over the phone and paid by a charge card.
